Please use this Intake Form to submit an expedited copyright infringement claim to DistroKid. After you submit this Intake Form, DistroKid will apply its standard process to review the information. If DistroKid is unable to process your claim as submitted or if your claim is missing information, DistroKid may need to contact you, to make sure you, the Claimant, are able to receive and respond to emails at the email address you submit below. If DistroKid receives an insufficient or invalid Intake Form from you, DistroKid may ask for any of the following: · A "confirmation" or reply email received by DistroKid directly from the email address you submit below · Your full legal name · Your full mailing address · The copyright owner's full legal name or business name (if you are not the copyright owner) · Metadata of your copyrighted work (if you are claiming a music copyright) · A more specific title or description of your copyrighted work · An additional or alternative active/working weblink that identifies the location and the specific material you claim to be distributed by DistroKid and infringing · Evidence that you are authorized to administer and/or act on behalf of a copyright owner (if you are not the copyright owner) · Confirmation or additional description of your evaluation of any exceptions to copyright, such as a limited license, fair use, or fair dealing SUBMITTING THIS FORM IS A SERIOUS LEGAL PROCESS. Fraudulent or abusive use of this Intake Form or any other violations of DistroKid's Terms (https://distrokid.com/terms/) may result in account suspension, account termination, or other legal consequences, including but not limited to damages.
